Dear Swim Team Parents,

The DeKalb Aquatics Swim Team staff would like to officially welcome you to the 2008-2009 swimming season. We have been working hard to get ready for our new season.

The season will officially start on Wednesday, September 3rd, at Agnes Scott pool. I am looking forward to seeing everyone along with the rest of the staff. You will find enclosed in this welcome packet the following items:
1) Membership Agreement

2) Medical Care Authorization Form

3) Swim Team Rules

4) Team Questionnaire and Goal Sheet
Please fill the forms out in their entirety and mail* the information to:

DeKalb Aquatics Swim Team, Incorporated
P.O. Box 2926
Decatur, GA 30031-2926.

All registration forms must be accompanied with the team registration fee. All existing swimmers should pay $80, and new members should pay $100. All fees including September swim fees and completed forms must be turned in prior to the first day of practice.

We look forward to seeing you September 3rd.

* New swimmers can tryout and register at 5:30 pm
* Returning swimmers can register at 6:30 pm
