Points of Understanding, Rules, Regulations & Disclaimer
  • DAQ utilizes the services of DAQ Treasurer for tuition and collection of all fees.
  • Parent/Guardian understands that the entire monthly membership fee is due by the 10th of every month according to the terms of this agreement whether or not the Member chooses to attend instructional sessions provided by DAQ. All late payments will be assessed a $25 late fee and NSF checks will be charged an additional $10 for a total of $35. If all outstanding fees are not paid by end of month, swimmers will not be allowed to swim until account has been satisfied in full.
  • Uniforms, accessories, meet fees, lodging, transportation and cost of meals are separate and not included in this agreement. Additional fees to enter competitive events are due upon request. If you sign up for a meet, then meet fees must be paid. Failure to do so will be addressed by the board of directors.
  • Parent/Guardian and Member agree to abide by all current rules and regulations governing the conduct and operation of instruction sessions in order to promote efficiency and safety. Parent/Guardian and Member agree to abide by all rules and regulations adopted, modified or hereafter amended.
  • DAQ reserves the right to revoke or terminate any membership and to change the hours of operation in its own discretion.
  • Parent/Guardian and Member certify that Member is in good health. All exercises and courses of instruction are taken at the Parent/Guardian and Member’s own risk. Parent/Guardian and Member fully understands that there is a risk of personal injury involved in this course of instruction and with this knowledge agrees to indemnify and hold blameless DAQ, its officers, instructors and agents from all losses caused by accident or injury to the Parent/Guardian, Member, or to third party persons.
  • DAQ, its officers, instructors and agents are not responsible for damaged, lost, or stolen articles.
  • Parent/Guardian and Member understand that this is a professional team and attendance is mandatory at practices and competitive events, unless a pre-authorized excused absence is granted.
  • All swimmers must be picked up no later than 15 minutes after practice has ended. All late pickups will be charged $5 per 10 minute interval rounded up. Failure to comply with this request will results in the following actions: (1) a verbal warning, (2) assessment of the fine, (3) assessment of the fine and a meeting with the Board, and (4) assessment of the fine and a suspension determined by the Board. .
